Insurance rates vary by your carrier and plan; you’ll need to contact your insurance to determine your out-of-pocket cost.
The codes we bill depend on multiple factors including the service and appointment type.
Your copay, coinsurance, or self pay rate are due at the time of service.
A payment method must be stored on file, and the copay is collected before each appointment.
If you do not have a card on file and/or the billing team is unable to collect your copay ahead of your appointment, then your copay will be collected by the front desk concierge when you check in for your appointment online.

No. Currently, Modyfi does not accept Medicare or Medicaid, cannot accept secondary insurance if primary is Medicare, and cannot accept self-pay for Medicare or Medicaid. Please keep checking back as this may change at some point in 2026.
No, even if you have a legal residence (“domicile”) and have established care in the state where your provider is licensed, you must be physically in the state where your provider is licensed while conducting the Telehealth session.
All vendors and systems used are HIPAA-compliant, Business Associate Agreements (BAAs) are in place with all providers and vendors, all stored documents are encrypted, and all staff and providers are required to use a high quality VPN.
Yes, medication providers can order labs as needed. They are also able to coordinate with you, other providers, and the lab to avoid duplication of lab orders.
Standard blood panels to functional lab testing, genetic testing, and other specialty labs (e.g. Genova, Mosaic) are available.
Modyfi medical and nutrition providers are able to review your historical labs with you, even if they were not ordered by a Modyfi provider.
Yes. After scheduling, you’ll get a confirmation and a link to complete intake forms from Tebra 3–7 days before the appointment.
Typically, the first visit is 60 minutes, and follow-up visits are between 30 and 60 minutes, depending on your preference, treatment plan, and medical necessity.

This is required so you do not need to wait long to check into your appointment. This payment method is used to cover your copay, co-insurance, account balances, and no-show fees.
Cancellation/reschedule within 24h or no-shows incur a fee:
- $100 for 60-minute therapy, 30-minute psychiatry/nutrition
- $150 for 60-minute psychiatry/nutrition
If a patient is >10 minutes late for a 30-minute or >20 minutes for a 60-minute appt, it’s considered a no-show.
Arriving >5 minutes late or ending >5 minutes early may incur a $30 fee.
