  • Insurance rates vary by your insurance carrier and plan, so you will need to contact your carrier to determine what your out-of-pocket costs will be, if any. The codes we bill for each session depend on a variety of factors.

    INDIVIDUAL & COUPLES THERAPY

    Therapy Initial Evaluation (60 minutes / 90791): $150

    Therapy Follow-Up (60 minutes / 90837): $125

    Therapy Follow-Up (30 minutes / 90832): $75

    PSYCHIATRY, NUTRITION, & MEAL PLANNING:

    Medication Management Initial Evaluation (60 minutes / 99205): $200

    Medication Management Follow-Up Session (30 minutes / 99213 + 90833 or 99214): $125

    Medication Management Follow-Up w/ Therapy (60 minutes / 99214 + 90836 or 99215 + 90833): $200

    GROUPS:

    All of the groups are shared medical appointments where both group content and treatments are discussed.

    DBT Skills Group (60 minutes / 99213 or 99214): $125 (1x/week, 8 week time commitment required)

    FOOD Group (60 minutes / 99213 or 99214): $125 (requires initial evaluation, 4 week time commitment required)

    Ketamine Group (60 minutes / 99213 or 99214): $125 (requires initial evaluation, satisfies therapy requirement for ketamine, 4 week times commitment required)

  • You will receive a link from Practice Better when you book your appointment. You will need to store a method of payment on file, and your copay will be collected after each appointment.

  • Use the Practice Better app or web portal to message your provider.

  • You can always view your upcoming appointments and appointment information in the Practice Better app or web portal.

  • No, unfortunately at this time we are unable to accept Medicare or Medicaid. We are also unable to accept a secondary insurance plan if you are covered by Medicare.

  • If you have a residence or “domicile” and have established care within the state that your provider is licensed, then you would be able to be seen while traveling, at college, etc, as long as you plan to return to your home state within 6 months.

  • All of the vendors and systems utilized for our services are HIPAA compliant with signed Business Associate Agreements (BAAs). All documents are stored electronically with a high level of encryption. To ensure HIPAA compliant communication please use the Practice Better app or web portal.

  • Yes. Your medication provider can order any appropriate labs needed for your treatment plan, and whenever possible will coordinate with other providers to ensure labs are not duplicated. Genetic testing and other specialty labs and tests are done through third party vendors like Genesight. You can order standard labs without insurance or a provider approval through Ulta Labs.

  • Yes. After you schedule your appointment you will be sent a confirmation, and then a link to complete your intake forms 3-7 days before your appointment. From the Practice Better patient portal you can pay any outstanding changes, see lab results, and communicate with your provider.

  • Typically, the first visit with your medication provider will be 90 minutes and subsequent visits will range from 30 minutes to 60 minutes depending on the treatment plan.

  • Your first visit with your therapist is 60 minutes. Subsequent visits are also 60 minutes. Some therapists offer 30 minute follow ups for those with shortened attention spans.

  • You will need Google Chrome or the Practice Better app on Android or Apple for your appointment. Stable Internet speeds of greater than 1MB upload and download will sustain the video and audio connection, though higher speeds are recommended.

  • Modyfi accepts all major credit cards and debit cards including those associated with HSA and FSA. No cash or checks.

  • A card on file is required in case the patient does not show up to their appointment or if there is a cancellation or reschedule 24 hours before the appointment time.

  • You can cancel or reschedule appointments 24 hours or more before the appointment time. Appointments that are ended early or started late by a patient may be subject to a fee.

    Cancellations or rescheduled appointments within 24 hours and no shows are charged $100 for 60 minute therapy appointments, 30 minute psychiatry appointments, and 30 minute nutrition appointments, and $150 for 60 minute psychiatry appointments, and 60 minute nutrition appointments. If a patient is more than 10 minutes late for a 30 minute appointment or 20 minutes late for a 60 minute appointment, then the appointment is considered a no show, and the no show fee will be collected. If a patient arrives more than five (5) minutes late to their appointment, or needs to end their appointment more than five (5) minutes early, a fee of $30 may apply.   • Yes. Parent(s) or legal guardian(s) must be present at appointments for patients under the stipulated legal age in your state. For example, a patient in Maryland has some autonomy over their mental healthcare at 12 years old. Prior to 12yo, and for some treatment plan decisions like med changes, the parent would need to be present and approve any changes to the treatment plan in session or before the changes were implemented.

  • Yes. Modyfi providers can collaborate with primary care and other specialty providers if the patient is willing to sign a release of information.

  • At this time Modyfi provides direct access to mental health therapists, psychiatric nurse practitioners, nutritionists, and disability and compliance specialists.

  • Yes. You will need to download the Practice Better app. The mobile device or tablet will need to be connected to the Internet, have a camera, and have a microphone.

  • New patients must first complete the New Patient Form and staff will reach out to get you scheduled. Existing patients can use the Practice Better app or web portal to schedule.
